OP-AA-108-101-1002 — Independent/Concurrent Verification
Purpose
Provides requirements for independent and concurrent verification of equipment configurations, including throttled valve positions.

Exam — 2023 Q67
Throttle valves (Section 2.7): Initial alignment or verification: fully close the valve, then open the required number of turns. For independent verification, the second operator may observe the first operator positioning the valve, then perform either an independent count of the turns open, verification of process variable, or verification of valve position for valves with incremental indicators. For throttled valves requiring independent verification, the second operator ACCOMPANIES the first operator (concurrent verification) — they do NOT go out alone afterwards as with typical independent verification of open/close valves.

Component Configuration Control (step 5.1.4.1.A): For 1 hour or less TSAS, an Abnormal Component Position Sheet (ACPS) is used to position the component, and the IV must be completed within 1 hour of entering the TSAS. For GREATER than 1 hour TSAS, a tagout may be applied as soon as practicable — do not confuse the >1 hour tagout rule with the ≤1 hour ACPS rule.
Exam — 2019 Q95
Component Configuration Control (OP-AA-108-101-1002): for a 1 hour or less TS REQUIRED ACTION (e.g., removing power to PORV block valve per LCO 3.4.5), configuration control is INITIALLY maintained with an Abnormal Component Position Sheet (ACPS). A WCD (Work Clearance Document) is hung as soon as practical AFTER the TS is entered — but the INITIAL configuration control tool for ≤1 hour TSAS is the ACPS.

IV waiver for Locked High Radiation Areas: per OP-AA-108-101-1002, the Shift Manager IS permitted to waive a "Hands On" independent verification of a manual gate valve located inside a Locked High Radiation Area when the estimated dose is greater than 10 mrem.
